Parish Verification Form


Our affiliated GRACE parishes contribute significantly to make Catholic education affordable, providing for reduced tuition via the Active Parishioner Incentive for families who are registered members in good standing who have students enrolled in grades K-12.

Within the 2024-25 enrollment process, the Standard Tuition Rate will be listed in the TADS Tuition Agreement for students enrolled in grades K-12.

  • Families are to complete the enrollment process in its entirety at the Standard Tuition Rate.
  • Following enrollment, the Parish Verification process is to be completed by the family with their GRACE affiliated parish.
  • Upon submission of the completed Parish Verification Form, the tuition rate will be adjusted in the family’s TADS Billing Account with the Active Parishioner Incentive.
    • Submitted Parish Verification Forms will be reviewed and processed no later than 30 days after receipt. A confirmation email will be sent to the family to confirm its review and processing.
  • Payment plans do not begin until July 1, 2024; therefore, submission of the completed 2024-25 Parish Verification Form before June 1, 2024 will ensure the tuition reduction is made in the family’s billing account.

The Parish Verification Form must be completed and submitted for each academic year to receive the Active Parishioner Incentive for students enrolled in grades K-12.

The Parish Verification Form and instructions are provided online in the TADS system during the enrollment process. The Form is also posted below. Families are responsible for contacting their GRACE parish to obtain verification and submit the form with the verification documented on it to GRACE.

! IMPORTANT: The deadline for GRACE families to submit their completed 2024-25 Parish Verification Form is June 1, 2024.

After June 1, 2024:

  • For Returning GRACE Families:
    • Verification will continue to be accepted until September 30, 2024. However, billing will commence and payments must be made as scheduled beginning July 1, 2024. At the time the adjustment takes place, the Active Parishioner Incentive will be applied to the account’s future balance.
    • Verification will no longer be accepted beginning October 1, 2024. If the returning GRACE family wishes to request an exception, a letter of explanation must be submitted with the completed 2024-25 Parish Verification Form.
  • For families new to GRACE who have their child/ren accepted and complete enrollment after June 1, 2024, the deadline to submit their completed 2024-25 Parish Verification Form is October 1, 2024 or 30 days from the date of enrollment if accepted for admission and enrollment after October 1, 2024.
